Here's a clean scan of the rear seat belt installation sheet for Mustang, Rotunda 1964. I got this in a Rotunda rear seat belt installation kit.

Trying to re-open this thread.
I am new to the forum. I am located in Sweden and have just bought a 65 Convertible. The car seems to be very close to the original and my plan is to keep it that way. Since the rest of my family likes to take a ride in the Mustang i would like to install rear seat belts. In this thread Ivygreen 65 mentions a 3 page instruction. Would it be possible for you to please email the instruction to:  perjohan.lundin@gmail.com ?  My plan is to use the anchor plates https://www.npdlink.com/store/products/mustang_anchor_plate_seat_belt_replacement_style-105168-277.html  that NPD is offering and have them welded in place and then also use the mounting kit that NPD is offering https://www.npdlink.com/store/products/mustang_mounting_kit_seat_belt_rear_seats-105160-548.html 

Would this be a solution that is as close as possible to what was done in 1965?

Any suggestions regarding reproduction seat belt kits that are close to the original design?

It would not have been normal for any dealer to weld in anchor plates on a '65 for rear seat belts.  The rear belt kits came with large washers to use to strengthen the anchor points.
