This was found under the rear carpet of GT/CS 8R01J167499 during restoration.  This is a two sided sheet, folded in the middle to make a small booklet. You can see the fold line in the photo.  I don't have a good copy of the build sheet found in the drivers seat springs which was also found.  Mike

Thanks for sharing Have a fair number of these. Only seem to find them when there is a car that failed something in the inspection. Some other versions (non-paint related) are referred to as "traveler sheets"
