I seem to recall seeing that type of personalization (initials) plate at places like Pep Boys, Western Auto, etc many many years ago.  I'm guessing a previous owner wanted to personalize his Stang.

Kind of remember the same cards with the emblem at the parts stores also though I though they were not completely rectangle - might be  same supplier different model/style

Think it was Cal Custom or possible Cal Customize that was the maker

Looked around on Ebay - could not find an old kit to grab a picture of   just a group of other dress up items from the period
