Posting for posterity and research.  Found on 5F07C63116x.  65A  C  28  12B  21  6  6.  Early 65 carpet with heel and toe pad & vinyl on rocker sides.  Black data plate.

Now if we could only get the carpet companies to make the correct heel pad.... :(
